Certificate of Enrollment for Learner’s Permit and Driver’s License

The DDS Certificate of Enrollment must be signed by a school official and notarized.

This cannot be done immediately and requires processing time.

The completed certificate of enrollment will be ready after 2 business (48 hours) after requesting this form.

You can request your DDS Certificate of Enrollment by completing the form. Students may also go to the main office and complete the student section on the form and return 2 days later to pick up the completed forms.
Alcohol and Drug Awareness Program (ADAP)
If your student took health class at EFHS and needs their ADAP – They should log into EADAP.org and sign into the account they used when completing the program and print their certificate. The ADAP form is only needed for obtaining your Driver’s License – not your permit.
Drivers Education Certificates of Completion can be obtained in the main office when they collect their DDS Certificate of Enrollment. These forms are only needed for obtaining your driver's license.
